Oasis Ski Brule Trip 2023

We will be going to Ski Brule as a youth group event departing from Jacob's Well at 6am and arriving back to Jacob's Well at 8pm.
Cost is $30 and includes Lift Ticket, Ski Rentals, Lessons and Lunch.
Please also provide money for dinner on the way back.
By filling out this from we will consider this your consent to the information below:
My child/children __________________________________, has my permission to participate in the Ski Brule Trip, as planned by the Jacobs Well Church Youth Group. I understand that this will include Driving to Ski Brule, breakfast on the way, lunch there, dinner on the way back and all day skiing/snowboarding. I understand that, unless previously arranged, I will drop off and pick up my child at Jacob’s Well Church before and after the event on March 3rd drop off 5:45am pick up 8pm.
Additionally, I give my consent to Jacobs Well Church Staff and Youth Leaders to seek all emergency medical care prescribed by a licensed physician for my child. I assume the responsibility for my child’s participation in Ski Brule Trip, and will not hold Jacob’s Well Church, nor its Staff or Advisors, liable for any illness, or injury incurred at Youth Group activities at which every reasonable precaution has been taken.
